Hormonal influences are the main cause of a particular kind of hyperpigmentation known as melasma or chloasma. It’s particularly common among women and is thought to occur when the female sex hormones estrogen and progesterone stimulate the overproduction of melanin when the skin is exposed to the sun.

Melasma occurs as a brown to brown-gray patches typically on the face but may also occur on the forearms and chest. The distribution is often symmetric and occurs primarily in pubertal girls and women although adolescent boys may also be affected. While melasma is usually idiopathic, it can be associated with pregnancy or the ingestion of oral contraceptives. Lesions tend to increase in size and degree of hyperpigmentation after sun exposure.

Pigmentation often wanes after pregnancy or discontinuation of oral contraceptives. However, treatment with potent topical sunscreens, bleaching agents (hydroquinone) or pigmented-lesion lasers may be helpful.

Melasma or chloasma spots are similar in appearance to age spots but are larger areas of darkened skin that appear most often as a result of hormonal changes. Pregnancy, for example, can trigger overproduction of melanin that causes the “mask of pregnancy” on the face and darkened skin on the abdomen and other areas. Women who take birth control pills may also develop hyperpigmentation because their bodies undergo similar kind of hormonal changes that occur during pregnancy. If one is really bothered by the pigment, the birth control pills should be stopped.

Changes in skin color can result from outside causes. For example, skin diseases such as acne may leave dark spots after the condition clears. Puffy eyes can have causes that aren’t due to underlying disease. Examples include aging, hereditary differences, allergies, fluid retention from large salt intake, trauma, or lack of sleep.

With aging, the tissues around your eyes, including some of the muscles supporting your eyelids, weaken.
